Care Coordinator

to join their dynamic team.

As a

Care Coordinator,

you will play a vital role in ensuring the smooth operation of care services.

Your responsibilities:

Coordinating and Scheduling Care:

Ensure clients receive the right care at the right time. Liaising with Stakeholders:

Work closely with clients, their families, and care staff to assess needs and develop customised care plans. Monitoring Care Delivery:

Ensure that care meets quality standards and regulatory requirements. Maintaining Accurate Records:

Keep detailed records of client care plans, progress, and any changes in requirements. Supporting Care Staff:

Provide guidance and resources to ensure exceptional care delivery. Collaborating with Professionals:

Work with health-care providers and community organisations to enhance the overall care experience.

Skills/Experience:

Previous experience in care coordination or a related role. Excellent organisational skills with the ability to prioritise tasks and meet deadlines. Strong communication and interpersonal skills

to effectively engage with clients, families, and care staff. Knowledge of care regulations

and industry standards. Problem-solving abilities and adaptability in changing situations. Proficiency in using computer systems and software for record-keeping and scheduling.
