We provide Community and Primary Care services to our local residents, in addition to a wide range of staffing, management and administrative support to our member practices. By working closely with our PCNs and other key partners, including the Local Authority and community Organisations, we help people to stay well and live healthier lives for longer. We operate Federation-led services including Respiratory and Spirometry hubs; Community Cardiology; Community Dermatology; Community Diabetes service; Access E-hub and Enhanced Access and we support PCNs with managing their NWL single offer contract of services and the National Network DES contract, including ARRS roles. We work collaboratively with other partners in the commercial sector including joint ventures; research and consultancy. we nurture and motivate so our people are enabled to reach their full potential Role Summary Care Coordinator (Health Inequalities) (HI) The Care Coordinator (HI) will lead administration tasks for the Health Inequalities team, supporting the team with the development, delivery and evaluation of HCL’s Health Equity Programme Plan. The key responsibilities of the role will be to: a) provide SystmOne and other administrative support for community health & wellbeing services; and b) provide project support to the Health Inequalities Team. SystmOne/Administrative Support: Provide essential administrative assistance to patient-facing community roles, including Health and Wellbeing Coaches, Community Health and Wellbeing Workers, and other similar services or pilots under the HI team’s remit. This ensures the smooth and efficient operation of these roles in delivering community-based support. Project Support: Assist the HI team with project coordination and delivery, enabling the team to effectively fulfil its mission to address health inequalities through innovation and collaboration in primary care. This position plays a critical role in supporting the HI team’s objectives and its impactful work in the community. Role Requirements and Responsibilities Responsibilities: 1. SystmOne and other administrative support for community services • To process referrals on behalf of practices, and community services on SystmOne in line with Stand Operating Procedures (SOP) • Act as a point of contact between practices and external community services when managing staff caseloads and clinical rotas. • Contacting patients to ensure appointments are utilised and rearrange appointments when necessary. • Assisting the service leads with SystmOne administration when requested. • Supporting the teams with SystmOne queries as they emerge. • Monitoring service emails and forwarding on to team members when appropriate. • Supporting teams with data consolidation for services using SystmOne where requested 2.

HI Project Support • Manage and execute administrative tasks, ensuring smooth operations across the Health Inequalities team. • Help to develop and support the delivery of project plans, ensuring all activities align with the team’s objectives. • Organise and participate in meetings, including drafting agendas, taking detailed minutes, and creating action logs with clear ownership and timelines, ensuring follow-up actions are completed. • Plan, prioritise, and organise personal workload effectively while supporting the coordination of team activities when required. • Standardise project management processes to improve efficiency and ensure consistency across the team. • Act as a point of contact for both internal and external stakeholders, addressing enquiries and maintaining productive relationships. • Compile project key performance indicators (KPIs) to support timely and accurate reporting. • Assist in gathering data and research relevant to projects and strategic initiatives. • Deliver a wide range of administrative support, including diary management, ensuring urgent matters are addressed promptly and escalated when necessary. • Respond to both internal and external enquiries, providing clear and effective communication. • Maintain productive working relationships within the team and across departments to support efficient service delivery. • Use judgment to address routine and non-routine challenges, referring complex issues only when necessary. • Develop and maintain efficient filing systems and track reporting schedules to ensure organisation and accessibility. • Represent and advocate for Health Inequalities initiatives when engaging with stakeholders. • Maintain an up-to-date register of collaborative projects within the team. • Perform other administrative and clerical tasks as delegated by the team. • Participate in and support face-to-face engagement activities that align with the objectives of the Health Inequalities strategy. • Follow HCL policies, standard operating procedures, and guidelines to ensure compliance. • Assist in organising, attending, and facilitating workshops or information sessions related to Health Inequalities projects.
