This is a part-time role (60 hours per fortnight) based in Wagga Wagga, NSW (Wiradjuri Country). In this role, you’ll work closely with clients, GPs, community nurses, specialists, medical staff, and Aboriginal Medical organisations to provide top-notch care for people dealing with chronic diseases and complex health challenges. In this role, you’ll take on key responsibilities that make a real difference in people’s lives, including: Coordinating high-quality care for individuals managing chronic diseases and complex health conditions. Collaborating with patients, families, carers, and the primary care team to create and implement tailored care plans that truly meet each client’s needs. Working closely with GPs and other medical professionals to enhance care planning and outcomes. Partnering with local health teams to streamline services and ensure seamless care coordination. Communicating clearly with patients, carers, and the community, ensuring they understand the information provided and actively seeking feedback to ensure it’s helpful. Enhancing access to vital services, from allied health and specialists to culturally appropriate resources, ensuring timely and affordable support for those with complex health conditions. To thrive in this role, you’ll need the following qualifications and skills: Registration as an Enrolled or Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) and/or as a qualified Aboriginal Health Worker. Proven experience in conducting comprehensive assessments, developing care coordination plans, delivering safe and responsive care, and evaluating outcomes. Strong understanding of the complexities involved in caring for individuals with chronic conditions and the ability to navigate these challenges effectively. Excellent communication and interpersonal skills, with the ability to engage and connect with Aboriginal and Torres Strait Islander communities, as well as people from culturally and linguistically diverse (CALD) backgrounds.
