CAMEO operates across our Foundation Trust via two established bases in Cambridge and Peterborough. This post will be located in the dynamic and multicultural city of Peterborough. We are seeking to recruit dynamic and enthusiastic Band 6 Care Coordinators: Nurses, Social Workers, or Occupational Therapists. The practitioner will provide specialist advice, assess, plan, and implement care to a designated caseload whilst working at all times to promote the safety and well-being of service users and their families/carers. The individual must be compassionate, committed, and have a good working knowledge of psychosis. The post holder will be an autonomous practitioner and responsible for a defined caseload of service users. The ideal candidate will have a strong teamwork ethic, sense of accountability, accompanied by an ability to work under pressure and with a desire for innovation. To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To work alongside the service user to develop their care plan along the recovery principles. To offer specialist assessment and advice where required. To assess, manage, and regularly review ongoing risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To ensure that clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies and procedures. To provide supervision of junior staff and trainees where appropriate (mentorship and preceptorship qualifications). To act within the relevant Code of Conduct dependent on your Registration Body. Education and Qualifications

Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role. Registered Social Worker OR Registered OT. Completion of Mentorship Course/ENB equivalent, or intention to complete. Evidence of continuous professional development. Experience

Significant post-qualifying experience in working with those presenting with serious mental illness. Experience of working alongside people with complex health problems in the designated care group. Experience of managing a caseload. Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems. Experience of conducting clinical assessments including risk assessments. Experience and knowledge of dual diagnosis/substance misuse. Previous experience of working in the community. Experience of supervising staff. Lived experience of mental health issues. Skills & Abilities

Excellent written and oral communication skills. Ability to develop effective professional relationships with others. Develop own skills and knowledge and provide information to others to help their development. Skills of assessing and interpreting service user conditions with appropriate actions. Plan and organise complex activities or programmes requiring formulation and review. Able to use Word, PowerPoint, clinical systems, email and/or willingness to learn. The ability to multitask and adapt to different unpredictable situations. Other
