The Older Adults Community Mental Health Team in Trafford is an integrated health and social care service that strives to provide quality recovery-based support to people with organic and functional mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the Older Adults CMHT operational procedure. Pay Enhancements – 30% additional for Evenings (8pm onwards) and Saturdays and 60% additional for Sundays and Bank holidays. 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years. Knowledge of the needs of Older Adult Service Users. Knowledge of the needs of Carers. Qualifications
Registered RMN. Further CPD/training in mental health skills etc. Mentorship of students. Experience
Experience working with Older Adults. Experience working in a Community Mental Health Team. Experience in care planning and risk assessment. Experience supervising others. Experience working within an MDT. Experience working with a team, as well as autonomously. Passion for working with Older Adults and their Carers.
