Responsibilities:

Coordinate and schedule appointments for patients Communicate with healthcare providers, patients, and their families to ensure proper care coordination Maintain accurate and up-to-date patient records Assist in the development and implementation of care plans Provide support and guidance to patients and their families regarding healthcare options and resources Collaborate with interdisciplinary teams to ensure continuity of care Monitor patient progress and make appropriate adjustments to care plans as needed Ensure compliance with all relevant healthcare regulations and guidelines Skills:

Strong organizational skills with the ability to prioritize tasks effectively Excellent communication skills, both written and verbal Proficient in using office software such as Microsoft Word, Excel, and Outlook Ability to work well in a team environment Attention to detail and accuracy in record keeping Knowledge of medical terminology is a plus Must be familiar with Care planner app and QCS. As a Care Coordinator, you will play a vital role in ensuring that patients receive the necessary care and support they need. You will work closely with healthcare providers, patients, and their families to coordinate appointments, develop care plans, and provide guidance on healthcare options. Strong organizational skills, excellent communication abilities, and proficiency in office software are essential for success in this role. If you are passionate about helping others and have the necessary skills, we would love to hear from you. Must be a driver as this role might require you to cover a carer in times of emergencies if needed. Job Types:
