Care Coordinator / Team Leader

to join our team and lead by example. The Role: You will play a key role in managing and overseeing the daily care requirements of residents while ensuring the highest standards of service delivery. Working closely with residents, families, and staff, you will maintain compliance with policies and procedures, manage a team of care assistants, and contribute to the overall performance and safety of the home. Key Responsibilities: Resident Care and Well-being

Build respectful relationships with residents and ensure care is tailored to their preferences and best interests. Monitor residents’ health and respond promptly to changes in condition. Manage daily care delivery, including personal care, medication administration, and care planning. Work with healthcare professionals and families to meet residents' needs effectively.

Team Leadership

Supervise and support Named Carers and Care Assistants, ensuring best practices are followed. Provide coaching and guidance to staff to maximize their performance. Conduct performance reviews, address poor practices, and maintain a positive working environment.

Policy Compliance and Implementation

Ensure compliance with Cedar Care’s policies, procedures, and systems. Identify and report any non-compliance issues and assist with the implementation of new policies.

Care Planning and Documentation

Develop, evaluate, and update care plans to reflect residents’ current needs. Ensure care plans are implemented effectively and communicated to all staff. Complete daily documentation related to residents’ health, well-being, and care activities.

Social and Leisure Activities

Work closely with the Wellbeing Coordinator to ensure residents’ social and leisure needs are met. Support staff in enabling residents to participate in activities that enhance their quality of life.

Safety and Emergency Management

Ensure the safety of residents, reporting any safeguarding concerns or incidents promptly. Take immediate and appropriate action in emergencies, including contacting healthcare professionals or next of kin as necessary.

About You: The ideal candidate will have: Proven experience in a senior care role within a care home or similar setting. Holding relevant Qualification in one of the below mentioned field of: Healthcare-Related Degrees

Nursing

(BSc in Nursing or equivalent) Health and Social Care

(BA or BSc in Health and Social Care) Social Work

(BA or BSW in Social Work) Public Health

(BSc in Public Health) Allied Health Professions

(Physiotherapy, Occupational Therapy, or Speech Therapy)

Management and Leadership Degrees

Health Services Management

(BA or BSc in Healthcare Management) Business Administration with a Healthcare Focus

(BBA with specialization in Healthcare) Specialist Degrees

(Mental Health Studies (BSc in Mental Health Nursing, Psychology or Sociology)

Strong knowledge of care planning, CQC standards, and compliance requirements. Excellent leadership, communication, and organizational skills. A compassionate and proactive approach to resident care. Job Types:
