Job Description: We are seeking a highly organized and compassionate Care Coordinator to join our team in Tunbridge. This role involves coordinating care plans and ensuring the delivery of high-quality services to clients, as well as occasional call-out responsibilities to address urgent needs.

Key Responsibilities:

Develop and manage care schedules to ensure efficient service delivery. Liaise with clients, families, and care staff to create and review care plans. Conduct assessments and ensure care meets regulatory standards. Monitor and address any issues arising from care provision. Handle urgent call-outs to provide support to clients or resolve issues as they arise. Maintain accurate records and ensure compliance with company policies and regulations. Work collaboratively with team members and external agencies to achieve positive outcomes.

Requirements: Proven experience in care coordination or a similar role. Strong organizational and communication skills. Ability to respond quickly and effectively during call-outs. Knowledge of care standards and regulations. Driving license and ability to travel locally for call-outs.
