We are seeking multiple candidates with a can-do attitude, to join our teams trust-wide, within Wirral, Chester and East Cheshire based inpatient areas.

Main duties of the job The Care Coordinator within an acute care ward will be responsible for coordinating aspects of patient care including liaison with other agencies both internal and external to CWP.

The post holder will be the central coordination point for multi-disciplinary activity, organising and supporting ongoing regular meetings to facilitate progress to recovery and discharge with relevant CWP team members, relatives, carers, and other external agencies.

Person Specification Qualifications

Able to demonstrate a good command of English language, verbal and written.

Knowledge of Office Administration/Customer service acquired through training or experience to NVQ level 3 or equivalent.

Knowledge and Expertise

Knowledge of Health and Social Care administration and processes.

Knowledge of Microsoft Office applications, internet and email.

Knowledge/familiarity with medical terminology.

Experience

Able to work well in a team.

Experience of working in an Office Environment.

Experience/knowledge of issues of working with confidential information.

Experience of working with healthcare professionals/previous experience in the NHS.

Working in a busy and demanding environment.
