You will work on a variety of duties, including: Deliver prescribed care to a defined group of patients/Service users that reflect their individual needs. Assess, plan and deliver, with the patient and their carer/family, care which takes account of patient choice and wishes. Contribute to a team approach to patient care in conjunction with all members of the Multi-Disciplinary team and liaise effectively with external professionals. Maintain accurate and contemporaneous records of patient/service user care delivered. Respect and apply the requirements of equality and diversity, promoting and role modelling these across the team. Develop and maintain a good professional relationship with patients, colleagues and visitors, responding promptly and courteously to requests, enquiries, concerns, complaints, and suggestions.
