The role will support the team to work across the county aligning with Neighbourhood Teams and Primary Care Networks to provide clinical support to Care Home residents. This enables them to support Care Home staff to deliver care to residents that is consistently of a high quality. The team needs to support and develop an evidence-based approach to service developments to aid Care Homes in delivering a range of initiatives that will both reduce avoidable hospital admissions and provide staff with the knowledge and skills required to manage residents' care needs locally.

The Practitioner will support the development, implementation, and evaluation of programmes and pathways of care and work autonomously to manage their workload, working across the county using a multi-disciplinary and multi-agency approach to embed high standards of care within the care homes in Somerset.

Main duties of the job The team will coordinate their work across the county to reduce the number of avoidable admissions to hospital from care homes and to improve the experience of care home residents when they become unwell by working collaboratively with Care homes so that residents receive the right care, at the right time, in the right place, by the right people.

Person Specification Qualifications Essential Educated to Degree level or equivalent. Evidence of continuing professional development. Management qualification or experience.

Experience Essential Experience of delivering services within a performance management framework and of developing and implementing action plans in order to meet specific national and local targets. Experience of service-based problem solving. Experience of working across organisational boundaries in a healthcare setting. Experience of providing high-quality effective services within resource limits. Experience of mentoring.
