Job Description:
We are looking for a compassionate and dedicated Care Manager to lead our palliative care team. The Care Manager will be responsible for overseeing the delivery of palliative care services, developing care plans, and providing support to patients and their loved ones. This role requires a high level of empathy, strong communication skills, and a commitment to enhancing the quality of life for individuals in palliative care.
Key Responsibilities:
Develop comprehensive care plans for patients in palliative care
Coordinate care services and ensure the delivery of personalized, holistic care
Provide support and guidance to patients and families throughout the palliative care journey
Collaborate with healthcare professionals to address the physical, emotional, and spiritual needs of patients
Monitor the effectiveness of care plans and make adjustments as needed
Ensure compliance with palliative care standards and ethical guidelines
Qualifications:
Degree in Health & Social Care or a related field
Experience in palliative care or end-of-life care settings
Strong interpersonal and communication skills
Ability to work collaboratively with a multidisciplinary team
Knowledge of palliative care principles and best practices
