We are currently seeking an experienced Care Manager to join our team. As a Care Manager, you will be responsible for managing a caseload of Clients living in the community with a range of complex care needs. The role includes assessing and updating care plans, supporting and supervising our Carers, auditing and managing documentation, liaising with external providers and professionals, and visiting your Clients on a regular basis to ensure that our high standards are met. We are seeking an individual with exceptional interpersonal skills to be the primary point of contact for clients and carers, ensuring a positive and supportive experience. Success in this role requires keen attention to detail and the ability to excel in a fast-paced environment. The ideal candidate will have proven experience managing challenging caseloads and possess some managerial experience, preferably within the healthcare industry.
