Case Manager/Discharge Planner Schedule:
Full-Time, 40 hours/week Monday-Friday, rotating Saturday coverage Base Pay:
$40-$43 an hour Licensing:
Must be licensed in the State of Massachusetts
Primary Responsibilities: Oversee and coordinate each patient's hospital stay from admission through discharge. Collaborate with the interdisciplinary care team to identify goals, address barriers to discharge, and ensure patients receive optimum rehabilitation services efficiently. Provide patients and families with information about the rehabilitation process, levels of care, and patient rights/responsibilities. Work within CMS guidelines and/or Managed Care expectations, advocating for patients when possible. Ensure patients are provided with appropriate resources for the next level of care (e.g., skilled nursing facilities, home health agencies). Maintain contact with patients and families regarding progress towards goals, care team discussions, and ongoing discharge planning efforts for safe and timely discharges. Address disposition changes promptly, proactively anticipating patients' discharge needs. Prerequisites: Registered Nurse with acute care case management experience. Excellent communication (written and verbal), interpersonal, and organizational skills. Ability to assess, analyze, plan, implement, and evaluate care plans. Basic knowledge of CMS guidelines, Managed Care products, and criteria for inpatient rehab vs. long-term acute hospital admission.
