Case Manager , you’ll play a key part in providing long-term case management to adults facing substance misuse challenges. Your responsibilities will include: Developing tailored care plans and delivering one-to-one support. Collaborating with the prescribing team to deliver harm reduction and specialist interventions. Supporting individuals to achieve stability, reduce harm, and regain independence in their communities. What You’ll Bring Relevant experience in the substance misuse field. A degree in Psychology, Sociology, Social Work, or a Diploma in Health and Social Care (or equivalent). Extensive experience in the field may be considered in place of formal qualifications, with access to accredited training.
