Job Responsibilities: Provide planned and unplanned care to patients and their carers in clinics, including one held at a village hall. Respond to urgent referrals from patients, carers, and the multi-disciplinary team. Triage and accept appropriate referrals directly from various sources and refer to other agencies as necessary. Assess, diagnose, plan, organize, and review complex patient needs, including advance care planning discussions. Ensure privacy, dignity, and human rights are respected for all patients and carers. Undertake comprehensive risk assessments and ensure the safety of nurses and carers. Monitor and maintain the use of medical equipment, ensuring proper maintenance and repair. Report and record all incidents related to health, safety, and security. Maintain accurate and confidential records, adhering to Trust Policies and NMC guidelines. Comply with infection control policies and report concerns to management. Manage time effectively based on caseload demands and balance patient needs with non-clinical aspects of the role. Act as a clinical advisor on healthcare issues and promote health across all age groups. Prescribe medications in line with Non-medical Prescribing policy. Identify community health needs and participate in projects, research, and audits. Serve as a role model for staff and students, demonstrating high standards of practice. Develop and implement standards in collaboration with other professionals. Work alongside the Community Matron and advance care practitioners to monitor complex health needs and reduce hospital admissions. Prioritize and delegate work within the multi-disciplinary team. Participate in Clinical Supervision as per organizational policy. Engage in annual appraisals and undertake necessary training for role fulfillment. Adhere to vulnerable adult and child protection procedures and organizational policies. Support the Team Manager in workload equity and deputize in their absence. Demonstrate awareness of resources and advocate for service users regarding eligibility criteria. Act as an authorized signatory for supplies and expenses. Track stock levels to ensure sufficient supply for service demands. Maintain effective relationships with the multidisciplinary team to provide quality patient care.
