NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. The North West London Integrated Care Board (ICB) is looking for individuals to join the service in a variety of roles and bands. The NWL team has come together under a new structure and this is an exciting opportunity to join a dynamic and positive team. North West London Integrated Care Board is looking to appoint one full time CHC Complex Clinical Lead for the Brent, Harrow and Hillingdon (BHH) locality team. The post holder will report to the BHH Head of CHC and Complex Care. The post holder will be responsible for leading on CHC Learning Disability, Court of Protection, Dispute Resolution, DOLs/LPS, complex case management issues, safeguarding, and high cost complex packages of care/placements. The role co-chairs panels with their Local Authority counterparts, and is the decision-maker at panel on behalf of the ICB. They will have oversight of the panel process ensuring that information presented to panel is in good order and that clear and clinically appropriate funding recommendations are being made.
Main duties of the job
We are looking for innovative, forward thinking, enthusiastic and confident registered health or social care professionals to be part of a skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare with sound knowledge and experience of the National Framework for NHS Continuing Healthcare is required.
The function of the continuing healthcare service (CHC) is to provide comprehensive and ongoing healthcare and support to individuals with complex, long-term health needs. Following being assessed as eligible for continuing healthcare, the eligibility outcome is based on the use of national frameworks and in line with the statutory responsibilities of the ICB for CHC.
The key objectives of the service include undertaking assessment, care planning, brokering care, monitoring and review of care packages, quality assurance of care providers, and providing an appeals process for individuals who have been assessed as not eligible for CHC. We will also ensure that people who have multiple care health and social care conditions are supported in an environment to keep them safe and provide high quality care.
