There is an expectation that the candidate would be in the office at least 2 days a week. You will be responsible for assessing children who may meet the eligibility criteria for NHS Continuing Care. This will include the assessment of care needs and the development, implementation, evaluation, and reviews of care plans for children. The post holder will work as part of Birmingham Children's Trust to support and advise social workers in completing Continuing Care assessments and understanding processes and decision-making. The post holder will develop the role and contribute to the implementation of a robust NHS Continuing Healthcare process for NHS funded care. Children requiring Continuing Care will have complex and/or intense health care needs, unstable fluctuating conditions, unpredictable health issues, or rapidly deteriorating health. They may exhibit aggressive disinhibited behavior that is difficult to control, unstable symptom control, or severe continuous symptoms, necessitating a high level of medical, nursing, or therapy intervention with monitoring from one or more health professionals.
