The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service offers an integrated, flexible, and family-focused service providing high quality, compassionate personalized support and care to Children and Young People across Stoke-on-Trent. You will be part of a team delivering the 0-19 Healthy Child Programme for a varied, unique, and diverse population, in supportive skill mix teams, working closely with our partner agencies across health, social care, and the local authority.

Main duties of the job The 0-19 Children's, Young People and Families Practitioner will work within uni/multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs. They will contribute to the delivery of the Healthy Child Programme (0-19) and to the improvement of the health and wellbeing of the population served. The nature of the service will be determined by local service specifications. The Practitioner will work in a variety of settings including clinics, Family Hubs, and schools, providing advice, information, guidance, signposting, and referrals.
