Are you an experienced Children's Nurse looking for a new challenge? We are looking to recruit to our Children's Community Nursing service where you will join an established supportive team here in Cornwall.
Due to service expansion and a future move to a 7 day working pattern, we are excited to be in a position to have a Band 6 Children's Community Nurse for Transition vacancy available, this role is county wide. This is an exciting new opportunity to focus on supporting coordination of the transition of children and YP from the Children's Community Nursing Services into adult services and supporting with complex discharge from acute care into the community setting whilst continuing to use your nursing and caseload management skills for care of children in the community.
Due to Maternity Leave the role will Initially will be focused on holding a CCN caseload whilst scoping the transition processes, following this period of cover the role will move to support the transition for the children and young people whilst continuing to hold a clinical caseload.
This is a permanent contract for 37.5 hrs currently divided over 4 days per week, to meet service need.
This role includes weekend and bank holiday working and occasional on call requirement for End of Life care.
Main duties of the job
You will have current experience in looking after acutely unwell children. You must be highly motivated and possess excellent acute clinical assessment skills and are able to communicate and organise effectively. You must be experienced in working under pressure and have the ability to work independently. We require your skills & enthusiasm to develop our strategy in supporting unnecessary hospital admissions and reduce length of stay by facilitating early and safe discharge from the acute trust as well as supporting children and young people with long term or palliative conditions. You will hold your own caseload, taking responsibility for planning, coordinating, delivering and evaluating care given.
