The successful applicant will be expected to: Provide high-quality nursing care for children in their own homes or alternative community settings. Have continuing responsibility for the management of a caseload, including continuing care packages. As a member of the Community Children's Nursing Team, ensure high standards of care by assessing, planning, implementing, and evaluating care in partnership with the child/young person and their family. Contribute to the development of the service across Hywel Dda Health Board. Act as a role model and expert practitioner. Offer clinical leadership and management of students and others who work within the Team. Facilitate an Integrated Children's Service. The ability to speak Welsh is desirable for this post.
