Job overview

Due to continued investment and development, we are looking to recruit two full-time Clinical Leads - Community Nurse working 37.5 hours per week (Weekends included and working day between 8am and 5pm included) to join our happy and friendly Community Health Support Team. We are a dynamic team who are proud to provide a high level of care and receive exceptional feedback in connection with our practice. The Team provides care to patients within the Western Cheshire Footprint, so travel is an important part of the role. Access to a car for work purposes is essential. The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership, and clinical oversight of the caseload. The team focus is on the prevention of unnecessary acute hospital admission and End of Life Care. By working collaboratively with the wider healthcare team, we ensure person-centred care for those patients whose preferred place of care is at home. A high level of communication is essential for the role, with teamwork at the core of our practice. Main duties of the job

Provide a highly visible clinical profile with leadership and clinical oversight of the designated Community Health Support Team caseload. Work collaboratively with the wider health, social care, and third sector partner agencies to meet the needs of patients. Ensure patients receive the right care, by the right person at the right time and the right place. Place the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care. Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives. Be aware of local public health community profiles and how this influences the required care for the population. Understand the skills required from the Community Health Support Team according to community profiles. Focus on the delivery of person-centred care. Provide effective, evidence-based care to a defined population through caseload management and monitoring of care standards. Provide clinical leadership. Working for our organisation

Qualifications

First level registered general adult nurse Evidence of teaching and assessing SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling, and application of health needs assessment Knowledge and Expertise

Proficiency in delivering high-quality evidenced based care Ability to demonstrate continuing professional development Ability to work and communicate effectively within a MDT setting Experience

Significant experience of working within a community healthcare setting Experience of making autonomous decisions You must have appropriate UK professional registration.
