Job Summary

We are looking to recruit an enthusiastic Clinical Lead to join the Ellesmere Port East Care Community Team. You will be joining a multidisciplinary team including but not limited to clinical leads, nurses, AHP's, and many other specialist professionals who visit the team.

We are looking for someone who is enthusiastic, self-motivated, and provides confident support and leadership to clients and staff. Good communication skills and an ability to work in partnership with service users and carers are essential.

You will also provide mentoring and support for team members and implement, monitor, and evaluate the effectiveness of the clinical standards within the team.

This post is part-time - 33 hours per week.

Main Duties of the Job

Provide a highly visible clinical profile with leadership and clinical oversight of a designated Care Community caseload. Work collaboratively with the wider health, social care, and third sector partner agencies to meet the needs of patients. Ensure patients receive the right care, by the right person, at the right time, and in the right place. Place the patient at the center of all decisions, reinforcing the concept of a more streamlined patient journey with minimal avoidable delays and an emphasis on people achieving independence while improving the quality of care. Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives. Be aware of local public health community profiles and how this influences the required care for the population. Understand the skills required from the Community Care Team according to community profiles. Focus on the delivery of person-centered care. Provide effective, evidence-based care to a defined population through caseload management and monitoring of care standards. Provide clinical leadership.

Person Specification

Qualifications

Essential

First level registered general adult nurse. Evidence of teaching and assessing. SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling, and application of health needs assessment. Knowledge and Expertise

Essential

Proficiency in delivering high-quality evidenced-based care. Ability to demonstrate continuing professional development. Ability to work and communicate effectively within a MDT setting. Experience

Essential

Significant experience of working within a community healthcare setting. Experience of making autonomous decisions.
