Job overview
We are looking to recruit an enthusiastic Clinical Lead to join the Chester South Care Community Team. You will be joining a multidisciplinary team including clinical leads, nurses, AHP's, and other specialist professionals. We seek someone who is enthusiastic, self-motivated, and provides confident support and leadership to clients and staff. Good communication skills and the ability to work in partnership with service users and carers are essential. You will provide mentoring and support for team members and implement, monitor, and evaluate the effectiveness of clinical standards within the team. This post is part-time - 33 hours per week. Main duties of the job
Provide a highly visible clinical profile with leadership and clinical oversight of a designated Care Community caseload. Work collaboratively with the wider health, social care, and third sector partner agencies to meet the needs of patients. Ensure patients receive the right care, by the right person at the right time and place. Place the patient at the center of all decisions, emphasizing a streamlined patient journey and independence. Provide direction within the caseload considering current health and social care agendas and local initiatives. Be aware of local public health community profiles and their influence on required care. Understand the skills required from the Community Care Team based on community profiles. Focus on the delivery of person-centered care. Provide effective, evidence-based care to a defined population through case load management and monitoring of care standards. Proficiency in delivering high-quality evidenced-based care. Demonstrate continuing professional development. Ability to work and communicate effectively within a MDT setting. Experience
