Job overview We are recruiting for a Clinical Nurse Specialist - Children & Young People Sickle Cell who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

You will use your specialist knowledge, experience, and skills to support the development and coordination of the haemoglobinopathy service and meet the needs of children and young people.

You will manage and coordinate clinical follow-up programmes working as part of a multidisciplinary team. You will act as a key worker for patients and facilitate the transition between child, young person, and adult services and will take a leading role in setting up nurse-led clinics in this area.

You will be expected and have experience with sickle cell disease and its management. This clinical responsibility requires an understanding of the potential medical and psychological needs of children and young people with haemoglobinopathy conditions.

Main duties of the job

To provide an accessible and robust service for all patients with Red Cell Disorders.

To support the ward and day-care staff with acutely unwell patients, liaising with the pain team, haematology, paediatric nursing, and medical team to ensure optimum care of the patient.

To manage the clinical caseload, providing highly specialist nursing input and continuity of care to patients with Red Cell Disorders, focussing on self-management, attendance, monitoring of their disease and treatment, as well as transition.

To ensure each patient is assessed, and their care planned, implemented, and evaluated in negotiation with the patient, carers, and other healthcare professionals accurately and ensure this is communicated and documented.

To provide expert nursing support and provide advice, education, counselling to support patients and families, either virtually, via telephone or face to face. Providing practical and emotional support to patients and families in a tailored and timely manner.

To meet with patients and their families as required to help resolve their concerns, providing written responses to formal complaints as required by the Divisional Manager.

To produce and update patient information and clinical guidelines for the service following trust guidelines.

To be involved in teaching and training of healthcare professionals and establishment of link nurses and to support the nurse haemoglobinopathy training programme.

Person specification Qualifications

Registered Nurse with the NMC – RSCN/RN

Teaching/Assessment program

Good knowledge & experience regarding safeguarding CYP

Experience

Minimum 3 years’ relevant post-registration experience in the same specialty

Undertaking or willing to undertake master’s degree / post-graduation qualifications

Skills

Demonstrates awareness of the importance of working in a multi-disciplinary team

Demonstrates understanding of good practice in Customer Care

Practical experience in teaching patients & staff

Experience of applying research evidence to clinical practice.

Skills in critical analysis and application of research to practice
