Job summary We are looking for a dedicated and highly motivated individual to join the already established Multiple Sclerosis team at Mid Yorkshire Teaching NHS Trust. You will be joining a long established, supportive and respected team of Neurology Specialist Nurses, who cover a range of neurological conditions. The team is friendly, forward thinking and committed to service development. You should have recent experience in the field of Neurology or experience nursing patients with long-term health conditions and a commitment to developing your knowledge and skills within the field of Multiple Sclerosis. Working both as a team member and independently are necessary requirements. You will be supported by and work collaboratively with Neurologists and Specialist MDT members ensuring effective assessment, planning, implementation and evaluation of care for patients with Multiple Sclerosis. The role requires excellent communication and influencing skills. Your expert intervention will lead to a reduction in unplanned hospital admissions, facilitation of early hospital discharge, readmission avoidance, a reduction in A&E attendances and ambulance call outs whilst providing holistic care throughout the patient journey. Main duties of the job The post holder will organise and manage a caseload for a defined group of patients/service users, providing specialist nursing support, advice, and education. Raise awareness of Multiple Sclerosis, reducing associated stigmas and promoting social inclusion. Reduce unnecessary admissions and facilitate early hospital discharge. Work towards practising as a non-medical prescriber within area of Specialty. Co-ordinate and manage nurse-led clinics within Specialty to include a relapse clinic and the review of patients treated with disease-modifying therapies. Responsible for providing evidence-based practice. Support the Band 7 CNS/team leader in leading identified aspects of the service. Ensure cost-effective use of all resources and appropriate governance management. Promote programmes of health education and health promotion relevant to the service. Review, interpret and act upon diagnostic investigations. Maintain accurate patient records within agreed databases and electronic systems. Provide mentorship and preceptorship as required to junior staff in the team or students on placement. Qualifications Essential Registered Nurse First level (Honours) degree or willing to undertake Evidence of continued professional development Recognised teaching qualification Post registration qualification in relevant subject Desirable Advanced communication skills Non-medical prescribing or willing to undertake Skills and Abilities Essential IT literate Teaching, assessment and presentation skills Organisation and negotiation skills Flexible attitude to working Ability to work autonomously and as part of a team Excellent communication skills Knowledge and Awareness Essential Insight into role Awareness of own limitations.
