A part time opportunity has arisen to join our specialist palliative care team. The 20.5 hour post to work over 3 days is a Band 7 CNS role based in Y Bwthyn Princess Wales Hospital Specialist Palliative Care unit. The position provides an outreach service to patients with palliative care needs across the community and hospital setting. The service provides support for the population of Bridgend and the surrounding valleys in the local authority area. The specialist palliative care service is delivered within teams on multiple sites across Cwm Taf Morgannwg. An appreciation that on occasions, dependent on patient need there may be a need for post holder flexibility in providing support to cover the CNS role across other sites. Main duties of the job

The position involves directly supporting and advising patients with palliative care needs in a hospital and community setting. Multidisciplinary team working to lead education and to provide support and guidance to colleagues about the specialty is key. The core hours are Monday to Friday 9-5pm but with participation in a 7 day roster with the same working hours which will require giving advice to other health care professionals, patients and families, potentially visiting patients in the hospital and community setting at weekends. The successful applicant would be responsible for managing a caseload of patients with specialist palliative care needs in collaboration with the wider MDT, participating in an education, audit and providing leadership regarding the specialty. You will need to be enthusiastic about the specialty, demonstrate experience of caring for patients with palliative care needs and be able to meet the travel requirements. About us

Qualifications

UK NMC Registered Nurse Qualification in palliative care at Degree/Diploma or equivalent skills, knowledge and experience, supplemented by specialist training, experience, short courses to Master's Level equivalent Teaching and assessing qualification Skills

Proven ability of practice development and innovation Experience

Significant post registration experience, with experience of caring for patients with palliative care needs Bereavement Care; Audit; Community nursing Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
