An exciting opportunity has become available for an experienced nurse to join our Specialist Palliative Care Team in Fife. The Specialist Palliative Care Team is a multi-disciplinary team who support patients with advanced illness who have complex and challenging physical, psychosocial, or spiritual needs. We are looking for a positive, highly motivated nurse to join us as a Clinical Nurse Specialist. You will have knowledge of Palliative Care, have completed an Advanced Assessment module, and be a Non-medical prescriber. There is an expectation that the successful candidate will undertake further palliative care education. The Outreach service provides Specialist Palliative Care to people in their own homes, care and nursing homes, community hospitals, and acute hospitals across the whole of Fife. Your remit includes leading and participating in daily huddles, discussing, and allocating both planned and responsive care to meet patient needs. You will contribute at the weekly Outreach Multi-Disciplinary Team meeting and be a source of professional advice and expertise to patients, families, and colleagues within the Health & Social Care Partnership and the Acute hospital. You will be expected to travel throughout Fife, working in all areas of the service, and it is essential you can travel in a timely manner. Our service is contacted by professionals through the Single Point of Access (SPOA), which is our central point, enabling immediate specialist advice for all areas of Specialist Palliative Care, including Outreach clinical advice and essential direct care, Acute Hospital and Allied Health Professional support, and Hospice inpatient referrals. You will assess, treat, prescribe, and direct further management, including signposting, referral to other care pathways, supporting discharges from inpatient to community settings, or liaising with colleagues when admissions to a variety of settings are required. We have a variety of visiting students and learners who access the service. As a practice assessor/supervisor, you will support individuals during their visits or placements. You will be part of a multi-disciplinary team and expected to take part in both continual professional development at an advanced level and service development alike. In collaboration with the Essential Care Charge Nurse, you will assist the Team Leader in ensuring day-to-day service delivery is maintained and provide support to junior staff across the service.
