We have an exciting opportunity for an experienced nurse to join the Neuro Rehabilitation Unit at Neath Port Talbot Hospital.

The post holder will provide specialist nursing care to people with neurological conditions who are referred into the Neuro Rehabilitation Unit.

The applicant will provide comprehensive specialist, multifactorial assessment, plan, implement and evaluate evidenced-based nursing care.

This role involves autonomous, independent clinical practice and working closely with Neurology consultants and clinical nursing staff within the department.

Provide specialist education and training to staff. This will include carrying a varied caseload of neuro-rehabilitation patients with a variety of presentations, multi-pathologies, severe physical and cognitive impairment.

Work collaboratively and cooperatively with multidisciplinary clinical colleagues to ensure comprehensive care to the patients.

The applicant is expected to undertake audits, support clinicians with their clinics, do ward-based work when needed, and undertake telephone consultations.

The post holder will be responsible for the development of the nursing role in the Neuro-rehabilitation service, junior nurses or AHPs, and rehabilitation assistants.

Main duties of the job

The post holder will be responsible for screening referrals, presenting at multidisciplinary team meetings, arranging admissions and discharges, and prioritising patients who are on the waiting list.

To assess, differentially diagnose, rehabilitate and manage a caseload of adults with neurological conditions.

To undertake the highly specialist assessment of neurological conditions, carry out diagnostic procedures and interpret the results for neurology patients with multi-faceted problems, including those with a highly complex presentation, and using clinical reasoning skills, knowledge of evidence-based practice and advanced specialist expertise determine appropriate care plans utilizing specialist skills and options.

Provide comprehensive education and support to patients and healthcare providers.

Participate in ward rounds, outpatient clinics, and multidisciplinary meetings.

Provide specialist clinical knowledge to clinical colleagues, patients, and carers/relatives.

Support a culture that promotes person-centred, safe and effective care.

Establish and maintain effective communication with patients and carers/relatives, and professionals across health and social services.

Seek appropriate advice/alert agencies when there are any concerns regarding potential/actual risk to any patient or their dependents e.g., Child Safeguarding, domestic violence, protection of the vulnerable adult, mental capacity, and deprivation of liberty.
