An excitingnew post has arisen as part of an exciting initiative. We are looking to recruit a Clinicalpharmacist who will work within the West Herts Enhanced Primary Care Monitoring service toprovide support and intervention to children and young people with ADHD between the ages of 6 and 18. Training and support will be provided. The postholder will be responsible for completing ADHD medication reviews utilisingspecialist skills and using clinical interview techniques to ascertain a childsresponse to medication and manage potential side effects. They will be expected to be confident inascertaining potential symptoms of co-morbidity such as ASD, mental healthneeds or physical health concerns and document information accurately and referto appropriate local services. You will beconsidering medication change that may be required to the child/young personsmedication care plan and discuss these with supervising GP and relaying thesecare plans to the patients records and GP.There will be GP supervision provided. Main duties of the job

This is a clinic based role, you will carry out holistic assessments of patients referred asappropriate by HPFT CAMHS into this primary care monitoring service. Applyknowledge of behavioural management strategies and evaluate responses to theseand medication. Ability to communicate orally, electronically and in writing withchildren, young people, and families. You will need the ability to report, analyse and discuss informationgained in your holistic assessment of the child or young person in the clinicwith your GP supervisor, psychiatrist or refer to appropriate services. Correspond with GP supervisors in the team, multi professional agencies,schools, families, children serviced and voluntary/ statutory services. Attend bi-weekly MDT meetings with senior teamto discuss and review caseload regularly. Knowledge of referral pathway for complex cases. About us

Manage andco-ordinate clinical pharmacist led ADHD clinics across South West Herts, providing updates to the supervising GP in relation to the management of any pharmaceuticalinterventions and to adjust drug treatment appropriately with the support ofsenior team and working within agreed protocols and guidelines with therecommendation of continued prescribing shared to the child/ young persons GPwithin recommended guidelines of the NMC medicine management code of conductand NICE guidance. Provide anintegrated service working closely with wider team and liaising with colleaguesin HPFT CAMHS and other services as required. As anautonomous practitioner, be responsible for monitoring patients with an ADHDdiagnosis on medication through clinical pharmacist led clinics, communicatingchange to other members of the team, and giving guidance on pharmaceuticaltreatment of Children and Young People up to the age of 18. The post-holderwill be able to take a holistic approach to care, considering a variety ofaspects which may impact on the childs function and behaviour and ensuring tohear the child's voice within their review process. Act as aspecialist resource within the service in respect of this client group workingautonomously to support families and other health care professionals. To attendvarious multi-agency meetings, write reports and plan care, acting as thepatient advocate when appropriate and to work in accordance with safeguarding childrenspolicies. Raise and discuss complexcases with supervising GP and ensure appropriate onward referral made. Promotepatient and public involvement activities in the specialist area, by involvingpatients, parents/relatives in the planning and delivery of care anddevelopment of services leading to service improvement. Contributeto the development of leaflets and information regarding medication and ADHDfor children, families, and professionals. Ensure theeffective and efficient use of physical and financial resources. Collect,collate, evaluate, and report information and maintaining accurate patient records. Ensure theservice is monitored on a regular basis and data is available as a record toprovide statistical information to Clinical Leads and others and to providedata to relevant local and national bodies to audit service as appropriate. Qualifications

Mandatory registration with General Pharmaceutical Council (GPhC) Skills Abilities and Knowledge

Knowledge of CYP legislation and relevant mental health legislation. Understanding of evidence-based practice and the ability to demonstrate how this influences clinical practice. The ability to assess, formulate and manage care appropriately, ensuring discharge is timely. To be able to function as a part of a multi-disciplinary team and to liaise effectively with other agencies i.e., education, GP's, maintaining positive working relationships. Ability to maintain effective clinical records both written and electronic. Ability to manage and organise own time and diary. An awareness of alternative and augmentative communication systems. Having a sound understanding of theoretical frameworks that underpin the understanding of behaviour Knowledge of children's mental health and Child Development Person and People Development
