You will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, and those with multiple long-term conditions. You will support the management of the practices' repeat and acute prescription management processes, as part of The Prescribing Team. You will review medication requests, issuing and arranging the necessary reviews as needed. You will work to improve communication about medication-related issues between the practice and other care providers. You will develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system. About us

As a more experienced Pharmacist, you will provide mentoring, training, and development for more junior colleagues. To ensure implementation of safe and efficient repeat prescribing systems. You will be part of a professional clinical network and will have access to appropriate clinical supervision as outlined in the Network Contract DES guidance where appropriate. You will support patients to take their medications to get the best from them, reduce waste, and promote self-care.
