County Durham & Darlington NHS Foundation Trust This is your opportunity to join our clinical pharmacy teams working across one of the largest acute care providers in the North East with positions available at both University Hospital North Durham and Darlington Memorial Hospital. You will also gain specialist clinical pharmacy experience in mental health through a rotational placement at Tees, Esk and Wear Valley NHS Foundation Trust.

All of our junior pharmacists are enrolled on the RPS Foundation pharmacist pathway and, supported by our dedicated junior pharmacist mentors, you will provide ward-based pharmacy services whilst ensuring that you progress against the four pillars of the framework. Formal training is provided through regular in-house teaching sessions and enrolling onto a choice of postgraduate diploma in a timeframe of your choice.

We are proud that the majority of our pharmacists are independent prescribers and therefore we will be able to support you with the IP qualification at the time point that it is right for you.

Main duties of the job All of our Pharmacists spend more than 90% of their time on Medicines Optimisation activities with minimal dispensary commitments. Our pharmacy technicians undertake much of our medicines reconciliation activities on our wards, allowing pharmacists to focus on excellent patient-focused clinical pharmacy.

The successful candidate(s) will be expected to deliver a high standard of near patient specialist pharmaceutical care to patients. They will support the pharmacy team to deliver governance and service improvement to their specialist area and will help shape the strategic direction of pharmacy by supporting the development of innovative new services. Experience will also be gained in cancer services and the dispensary.

Job responsibilities

Participating in the structured rotational programme, to include aseptic and community services across the sites as necessary.

Participating in the Foundation Level competency framework for Band 6 pharmacists.

Providing medicines management, medicines optimisation and medicines reconciliation services to help achieve department priorities and targets on a designated ward or group of wards as decided by the Lead Pharmacist.

Helping integrate the role of medicines reconciliation into the existing medicines management framework.

Helping implement local policies and guidelines on good prescribing practice.

Influencing local practice to ensure safe medication use, and to ensure that their own practice and the practice of others effectively acknowledges good prescribing practice, patient safety and risk minimisation.

Being a point of contact for nursing and clinical staff regarding medicine management issues.

Providing medicines information to healthcare professionals utilising the range of information resources available, including the library and the internet, and to make an informed decision on the appropriate response required.

Being involved in providing clinical pharmacy service to designated multidisciplinary team structures e.g. outpatient clinics, pre-assessment.

Providing high quality patient education on medicines use in both in-patient and outpatient settings.

Engaging in activities relating to clinical effectiveness including: drug use review, clinical audit and clinical self-audit and to contribute to ongoing measures of service evaluation.

Acting as a professional checker both in the dispensary and/or aseptic service.

Taking part in the on-call rota.

Person Specification Special Skills & Knowledge

Good understanding of current pharmacy/NHS issues.

Knowledge/experience of clinical pharmacy activities.

Ability to apply theoretical knowledge to ward-based situations.

Evidence of patient-facing clinical pharmacy activities.

Ability to answer complex queries through a sound knowledge of MI resources.

Supervision of others.

Qualifications

Masters Degree in Pharmacy or equivalent.

Evidence of CPD and education.

Statutory Registration

Registered as a trainee pharmacist or practising Pharmacist with the GPhC.

Membership of the RPS &/or UKCPA.

Special Requirements

Good oral and written communication skills.

Good interpersonal skills.

Able to build good relationships with colleagues.

Team player.

Desire to develop clinical skills.

Able to prioritise own workload.

Self-motivation and ability to motivate others.

Evidence of presentations and/or teaching.

Experience

Sufficient experience to provide near-patient clinical pharmacy and dispensary services.

Experience of developing good multi-disciplinary team working with other health care professionals.

Experience of Hospital Pharmacy.
