Retford & Villages Primary Care Network in partnership with Kingfisher Family Practice are looking for a motivated and enthusiastic Clinical Pharmacist to be an integral part of our multi-disciplinary team, driving positive changes in healthcare for local people. The Primary Care Network (PCN) model enables a new and different way of working, with a different mix of skills in primary care and refreshed focus on improving the health of our population. Main duties of the job

To undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. To carry out medicines optimisation tasks including effective medicine administration, supporting the Clinical Pharmacists structured medication reviews and medicines reconciliation. To support care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies. To utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. To provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities. To manage shared care protocols and liaise with Clinical Pharmacists for more complex patients. To support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally. To support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed. About us

Kingfisher Family Practice has over 15,000 patients and strives to be a great place to work while providing the best care possible to our patients. Retford & Villages PCN comprises of 5 General Practices: Riverside Health Centre, Kingfisher Family Practice, Crown House Surgery, Tuxford Medical Centre and North Leverton Surgery. With a population of over 54,000 people, we are largest of the three PCNs in Bassetlaw, responsible for 44% of the registered patients. The area we cover is largely rural. Although the area served is relatively affluent, there are more deprived families and communities clustered around the town of Retford. Job responsibilities

To work as part of the primary care team in the practice supporting the team to help achieve efficiencies and effectiveness. To support with leading improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the PCN. To improve repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. To implement efficient ordering and return processes and reducing medication wastage. To provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS). Promotion of Electronic Repeat Dispensing (eRD) and online ordering. Develop relationships with other pharmacy professionals and members of the multidisciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health. Support practice reception teams in streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. To contribute to the PCN delivery of the highest possible quality standards, the NHS Long Term Plan, Quality and Outcomes Framework, Investment and Impact Fund and locally commissioned enhanced services. To support the PCN in reviewing and developing practice policies for CQC requirements. To develop a culture that promotes equality and values diversity. To ensure that any infection prevention and control issues are reported to the line manager/Infection Prevention and Control. Person Specification

Qualifications

Masters degree in pharmacy or equivalent. Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge. Registered with the General Pharmaceutical Council. Experience of working within primary care. Qualified prescriber. Experience

Minimum of 2 years experience as a pharmacist, in acute hospital, community pharmacy or community health services, demonstrated within a practice portfolio. Experience of being responsible and accountable for own professional actions, working independently within professional and organisational boundaries and being responsible for the actions of others within their area of responsibility. Experience in supporting with the planning, development and delivery pharmacy services. Relevant theoretical and practical knowledge of Primary Care Networks, General Practice and evidence based medicine. Knowledge of NHS Long Term Plan agenda and Government targets. Awareness of systems to support management of patients in a primary care setting, delivering pharmaceutical input and support in the context of pathways of care and the business of the organisation(s).
