Clinical Pharmacist for GP Practice & PCN - Remote Virtual Pharmacist Ltd is at the forefront of delivering bespoke, dynamic healthcare services with over 20 years of combined expertise within the NHS, primary care, and pharmacy sectors. We invite experienced Clinical Pharmacists to join us on a contractual basis, offering the opportunity to work remotely, manage your schedule, and contribute to our dynamic healthcare services. Role Requirements: Essential: GPhC registration Independent Prescriber Status (preferred) Minimum of 2 years Post Graduate experience in a GP Practice Proficiency in using SystemOne or EMIS Completion of the CPPE pathway Strong clinical leadership, communication, and interpersonal skills Desirable: Experience in running chronic condition clinics Proficient in structured medication reviews and acute medicines request reviews What We Offer: Competitive Rates: Negotiable, based on experience - Current max rate £30 per hour Flexibility: Control over your work, schedule, and location Professional Development: Access to resources and support for continuing professional development (contractors responsible for their own CPD) Support Network: Access to a network of pharmacists and senior pharmacists Equipment: Provision of secure and necessary IT equipment for service access Our Values: At Virtual Pharmacist, we put the patient at the heart of everything we do. Key Responsibilities: Conduct comprehensive medication reviews to optimize patient treatment plans Review and manage acute medication requests with a focus on patient safety and efficacy Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care Undertake additional tasks as needed to support the healthcare team and patient care initiatives Application: If you share our values and are looking for an opportunity to grow professionally while maintaining a flexible work-life balance, we would love to hear from you.
