Main Duties of the Job
You will work as part of a multi-disciplinary team in a patient-facing role.
Manage repeat prescription requests, resolving queries where possible within the scope of practice, and ensure patients have appropriate monitoring tests in place when required.
Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage, and other local or national guidance.
Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
Undertake a proactive role in audit and quality improvement, implementing recommendations where appropriate.
Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
Participate in the review and setting of policies relating to medicines management and prescribing, ensuring practices prescribe in accordance with local guidelines and formulary.
Identify areas of clinical risk at the network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources.
Monitor network practices prescribing against the local health economy and make recommendations for GPs or amend within the scope of practice based on findings.
Maintain own clinical and professional competence and be responsible for own continuous professional development.
Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.
About Us Manor View Practice is a well-established PCN located at 5 sites in Bushey, Watford, South Oxhey, and Borehamwood, with a list size of 45,000+ patients. We are looking to welcome an experienced Clinical Pharmacist into our PCN who will play a full and active role as part of the practice's multidisciplinary team.
The practice has a team of 6 GP Partners, 32 GPs, 10 Pharmacists, 2 Advanced Nurse Practitioners, 3 Paramedics, 6 Practice Nurses, 5 Healthcare Assistants, and 2 phlebotomists. The surgery received a rating of 'Good' following its most recent CQC inspection.
Person Specification Qualifications
Completion of Pharmacy Degree registered with a GPhC.
Excellent Clinical and Medication knowledge.
Understanding and experience of working as a pharmacist in a GP practice setting, with a confident approach.
Experience of undertaking medication reviews.
Commitment to continue to learn and develop.
Ability to be adaptable and flexible and be a proactive problem solver.
Excellent inter-personal patient-facing communication skills.
Registered with the General Pharmaceutical Council.
Will hold Personal Professional Indemnity Insurance.
Completion of CPPE PCPEP pathway.
