The salary for this permanent Clinical Pharmacist job is £42,186 - £49,160 per annum. Main Duties: Collaboration with the pharmacy technical team to undertake medicines reconciliation on admission and assessment of patients' own medication for use during their inpatient stay. Review of medication on admission to ensure medicines are safely and appropriately prescribed and patients' own medicines or alternative supplies are available for administration. This may involve making judgements on the suitability of patient’s chronic medication regimen in the hospital situation when the patient’s condition may have changed. Working within the Medicines Management Policy and procedures to ensure safe and cost-effective medicines use. Undertaking regular reviews of inpatient prescription charts to ensure any amendments or additions to medicine regimes are safe, effective, and appropriate. Discussing prescribed medicines with medical staff within primary and secondary care to ensure the patient receives safe, effective, and appropriate treatment during their inpatient stay and on discharge. Monitoring patient’s biochemistry and haematology results; assessing medication throughout the patient’s stay in hospital to ensure doses are appropriate and adverse effects are minimised. Monitoring the administration of injectable medication as bolus injections, by infusion pumps, syringe drivers, and PCAs, ensuring doses and administration routes are safe and appropriate. Planning and implementing discharge procedures to ensure the safe and effective reconciliation of medicines for continuation of therapy and review as necessary after discharge. Provide counselling and advice on medicines use to patients and carers (verbal and written) to assist patients in improving compliance and using their medication to maximum benefit. Liaise with multi-disciplinary teams, including community and primary care pharmacy services, to ensure effective continuity of pharmaceutical care across all healthcare sectors on admission and discharge. Advise on and facilitate the provision of Total Parenteral Nutrition, with support from senior pharmacists and dieticians. Requirements of the Clinical Pharmacist: An undergraduate Master’s Degree (or equivalent) in Pharmacy. Pre-registration training and successful examination (1 year). Pharmacist registered with the General Pharmaceutical Council (Practising). 1 or more years post-registration experience in hospital or community pharmacy. Detailed knowledge and understanding of pharmacy legislation. Good IT skills.
