Job summary The South Lincolnshire Rural, Primary Care Network is offering an exciting opportunity for a Clinical Pharmacist to work within our Holbeach Park Road, GP practice. Our Primary Care Network (PCN) recognizes the value that pharmacists can bring to our practices and our patients, and we look forward to growing our PCN pharmacy team. Our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients. We are seeking an enthusiastic and forward-thinking pharmacist to join our like-minded PCN. You will become part of a group of PCN Clinical Pharmacists working under the supervision of the Lead PCN Clinical Pharmacist and GP Clinical Supervisor. Primary care experience would be an advantage; however, it is not essential. Flexibility, enthusiasm, and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor, other clinical pharmacists, and enrolment onto a formal education pathway delivered through CPPE (if not already complete). Main duties of the job Review and interpret medication orders to ensure accuracy and appropriateness for individual patients. Monitor patient responses to medications and make necessary adjustments in collaboration with the healthcare team. Be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice team. Provide drug information and education to healthcare professionals and patients regarding medication therapies. Participate in multidisciplinary rounds, offering expertise on pharmacotherapy to optimise patient outcomes. Conduct medication reconciliation during patient admissions, transfers, and discharges. Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD, and asthma) and people with learning disabilities or autism. Ensure compliance with all relevant regulations and standards related to medication management. Maintain accurate records of medication use and interventions in accordance with hospital policies. We work collaboratively as a team, which includes GP's, Social Prescriber, Specialist Neighbourhood Nurse, and Pharmacy staff. We work closely with local agencies and providers of support services. SALARY: £50,000 - £55,000 Primary Duties and Areas of Responsibility Patient facing Long Term Conditions See patients with single or multiple medical conditions, where medicines optimisation is required. Review the ongoing need for each medicine, ensure that monitoring is up to date, and support patients to get the most benefit from their medicines. Make appropriate recommendations to Senior Pharmacists or GPs for improvements in treatment regimes. Patient facing clinical medication review. Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring. Patient facing care home medication reviews Work with care home staff to improve the safety of medicines ordering and administration. Patient facing domiciliary clinical medication review Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacists, nurses, and GPs on prescribing and monitoring. Management of common / minor / self-limiting ailments Managing caseload of patients with common/minor/self-limiting ailments while working within scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate. Patient-facing medicines support Provide patient-facing clinics within your base practice for patients with questions, queries, and concerns about their medicines. Telephone medicines support Provide a telephone helpline for patients with questions, queries, and concerns about their medicines. Medicines information to practice staff and patients Answer relevant medicines-related enquiries from GPs, other practice staff, other healthcare teams (community pharmacy), and patients. Suggest and recommend solutions. Unplanned hospital admissions Using prescribing support tools such as Eclipse and PINCER, review the use of medicines most commonly associated with unplanned hospital admissions and readmissions, through audit and individual patient reviews. Management of medicines at discharge from hospital Reconcile medicines following discharge from hospitals, intermediate care, and into care homes. To review medications for newly registered patients Experience Essential Have experience of or an awareness of common acute and long-term conditions that are likely to be seen in general practice. Must hold or be willing to work towards an independent prescribing qualification. Recognises priorities when problem-solving, and identifies deviations from normal patterns; is able to refer to seniors or GPs when appropriate. Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct. Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines. Desirable Working in Primary Care. Working in a GP practice. Qualifications Essential Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. Must hold or be willing to work towards an independent prescribing qualification. Must have completed/must enrol on the CPPE Primary Care Pharmacy Education pathway (PCPEP).
