About the Role Stevenage South Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a part-time basis, for 30 hours per week. The successful applicant will be responsible for implementing an effective medicine management system within the practices, including cost-effective prescribing, working as an independent prescriber (or willing to work towards independent prescribing status), offering prescribing advice and support for both patients and colleagues, and conducting long-term condition reviews. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the PCN. Key duties include: Reviewing the on-going need for each medicine for patients with single or multiple medical problems where medicine optimisation is required, including review of monitoring needs. Making appropriate recommendations to peer PCN/practice pharmacists or GPs for medicine improvement. Undertaking clinical medication reviews with patients and producing recommendations for the senior clinical pharmacist, nurses, or GPs on prescribing and monitoring. Managing a caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Reconciling medicines following discharge from hospitals, intermediate care, and care homes, including identifying and rectifying unexplained changes. About the Candidate You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications: A pharmacy degree and registration with the General Pharmaceutical Council. Post-qualification experience in pharmacy (hospital, primary care, or community), or working towards obtaining this. Qualified independent prescriber, or working towards this (the network will support the successful candidate to work towards this). Member of the Royal Pharmaceutical Society (RPS) desirable. At least 3 years’ post-registration relevant experience. Experience working within primary care is an advantage. About Us Stevenage South PCN (SSPCN) is made up of local practices who are commissioned to work together to provide a range of healthcare services to a population of 60,000 patients across Stevenage. The core network practices include Bedwell Medical Centre, King George and Manor House Surgery, Knebworth & Marymead Medical Practice, and Shephall Health Centre; however, other practices may join the PCN in the future. The PCN is governed by a PCN board, with representatives from each practice, who provide oversight of the service and approval for strategy and future PCN plans and objectives. SSPCN has a fantastic work ethic and strongly believes in supportive teamwork.
