The Clinical Pharmacist will complement our existing pharmacy team proactively helping patients across the PCN to stay safe, well, and out of hospital whilst supporting efforts to reduce the demands placed on general practice. You will be a leading example of clinical excellence, supporting colleagues in the PCN, and providing an advanced level of experience and care. The post holder will help patients to get the best from their medicines, including: Annual and acute medication reviews Switching medications to agreed and approved protocols Improving repeat prescribing processes in General Practice Promotion of repeat dispensing and online ordering Minimising clinical risk and reducing wasted medicines The clinical pharmacist will be directly responsible for care and prescribing of defined cohort(s) of patients and must have completed or be near to completing training to become a prescriber to work alongside the general practice team. They will deliver and develop pharmacy-led medicines optimisation, ensuring each practice remains up to date. Ensure best value, reduced harm, and improved outcomes by working to the principles of Medicines Optimisation and relevant up-to-date national guidelines and evidence base. Support the Prescribing Lead with the medicines agenda within the PCN and primary care practices. Administration

Contribute and participate in audits, evaluation, and clinical standard setting within the PCN and the 4 Practices. Responsibilities include: Accurate summarising of patient records and read-coding patient data Completing all required paperwork for legal and administrative purposes Ensuring that all practice policies are fully implemented About Us

The Crane Valley PCN is a group of GP practices (The Barcellos Family Practice, The Cranborne Practice, Verwood Surgery, and West Moors Village Surgery) located in beautiful East Dorset. This position is to work directly within the Cranborne practice as part of the wider PCN team, with a site in Verwood and Cranborne. Job Responsibilities

Work in accordance with all governance and internal systems relating to the management of clinical data and systems. Responsibilities include: Taking responsibility for own development with evidence-based knowledge and competence Staying up to date through attendance at courses and study days Working closely with other clinical staff and administrative managers Delivering the NHS contract requirements related to the practice Receiving supervision sessions by a senior clinical pharmacist Being assigned to a GP clinical supervisor for support and development This job description is not intended to be exhaustive and may be changed after consultation with the post holder.
