Clinical Practitioner Role

working towards the 4 pillars of Advanced practice within Care of the Elderly Medicine:

Clinical:

assessment/ diagnostics/treatment/ discharge/ complex communication with patients, relatives, and peers. Education:

delivery of education, support, and mentorship to MDT around frailty. Responsible for own development and education needs. Maintain live portfolio of evidence around Advanced Practice. Research:

carry out Audit/QI/service evaluation/ research in relation to practice. Leadership:

lead on service change, contribute to key HB strategy development groups. We are looking for a highly motivated and enthusiastic Clinical Practitioner to join our Acute Frailty/ Care of Elderly Medical team. The candidate must be working towards/willing to work towards an MSc in Advanced Clinical Practitioner and Independent prescribing qualification. They will be expected to work autonomously making higher-level clinical decisions for patients in ward-based areas, acute admission services, and outpatient frailty services. The post holder will work within the context of a MDT and will need to demonstrate robust leadership and management skills in the context of the role. Research and the delivery of education to a multi-professional audience will also be an expected part of the role. We welcome applicants from Nursing, Physiotherapy, and Paramedic backgrounds. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Cwm Taf Morgannwg University Health Board is part of the NHS Wales family.
