We are seeking a highly skilled and experienced healthcare leader to join us on a full-time basis as a Band 8a Clinical Service Manager/Clinical Lead for the Community DGS Virtual Ward. This is a senior leadership position responsible for the overall management, coordination, and development of our Virtual Ward service, which is a key model for delivering safe, effective care to patients within the community. You will oversee a team of experienced senior nurses and Health Care Assistants, ensuring the effective use of resources within the allocated budget while maintaining high standards of care. The Community DGS Virtual Ward is integral to reducing hospital admissions and lengths of stay by providing high-quality, acute care in the community. As part of a growing service, you will ensure that care delivery is aligned with national guidance and tailored to meet the needs of the local population. This is an exciting opportunity to lead an innovative and evolving service, improve patient outcomes, and shape the future of community healthcare. An Independent Non-Medical Prescriber qualification is essential for this role, along with a driving license and access to a car for work purposes. Main Responsibility

As a Clinical Service Manager/Clinical Lead your responsibilities will involve: Team Leadership & Management : Operationally and clinically manage a team of Nurses and Healthcare Assistants, overseeing staffing, recruitment, retention, and budget management for the Virtual Wards. Technology Integration : Lead the use of technology in Virtual Ward care to enhance patient outcomes and service delivery. Service Improvement & Change Management : Implement changes in service delivery based on local/national priorities and governance approval. Partnership Working : Collaborate with local system partners to develop integrated care models. Clinical Leadership : Provide leadership with a focus on clinical excellence, adhering to NMC standards and community/acute nursing practices. Addressing Issues : Proactively manage clinical, staffing, and performance issues, ensuring high-quality care and a motivated team. Patient-Centered Pathways : Develop pathways to prevent avoidable hospital admissions, ensuring seamless care across primary and secondary care services. Collaboration & Communication : Build and maintain strong working relationships with multidisciplinary teams, local services, and patients to ensure integrated care delivery. Risk Management & Decision-Making : Monitor staff allocation, identify risks, and make autonomous decisions to ensure service continuity during periods of increased demand. Workforce Development & Continuous Improvement : Lead staff development, ensuring training, clinical supervision, appraisals, and compliance with mandatory training, while fostering a culture of learning and evidence-based practice. The Ideal Candidate

Our ideal candidate will have: Registered Nurse, current registration with the NMC Registered Independent Non-Medical Prescriber Relevant postgraduate Community Nursing/Acute Nursing specialty qualification/training Minimum of 5 years post-registration experience, including at least 2 years working in a supervisory/managerial capacity at minimum band 7 level within a community/acute nursing setting Evidence of enhanced clinical practice at Masters level in relevant specialty Experience of effective budget and resource management A driving license and access to a car for work purposes are required for this role.
