Job summary We are looking for an enthusiastic, hard-working, quality-driven individual to help support our GP Surgery staff in providing a high-quality medical service. Main duties of the job The Clinical Services Co-ordinator will provide the day-to-day operational management of the team for a smooth running of patient-facing services. The post holder will manage the synergy between various practice teams and effectively co-ordinate with primary care support teams such as the Primary Care Network (PCN). Job description Job responsibilities Plan, support, facilitate and monitor progress of Clinical Services across the GP Surgery. Demonstrate knowledge and skills in use of various software within the GP Surgery, such as EMIS Web, Docman, AccuRx, Pharma Outcomes. An exciting opportunity has arisen for a Clinical Services Co-ordinator in the organisation, who must be available to work on a full-time basis with flexibility when required to work across the GP Surgery opening hours, Mon-Fri, 8am-6.30pm. Job responsibilities Facilitate and ensure the effective delivery of patient-centred, personalised health and social care plans for patients, monitoring progress and reporting outcomes, contributing to patient reviews and care planning within appropriate time frames. Explain the management of a patient's pathway to clinical staff, liaising between service teams and service users, contacting services using the appropriate procedures/referral mechanisms. Work closely with all relevant care agencies (primary care, secondary care, community services, Mental Health, Social Services, Ambulance Service, Voluntary services, and other relevant service providers) to ensure a smooth delivery of patient services. Maintain accurate records and statistical returns as required by the ICB, including providing patient-related information for entering Clinical Reporting Systems, within the required time frame, such as QOF for EMIS Web. Collect data on patients/carers for recognised outcome measures and document for service interpretation. Ensure all patient notes are updated to reflect any changes, including details on plans. Managing operational meeting processes, identifying patients for discussion, and working closely with clinicians to define and plan service delivery. Organise and attend relevant meetings when required including PCN meetings. Ensure that meeting actions are recorded, disseminated, and followed up in a timely way; ensure relevant practitioners are aware of meeting decisions and actions/outcomes, and chase for action resolution and update. Be a contact point for GP Surgery staff, helping the team establish systems and processes which will ensure a timely and appropriate response to queries from clinicians and other stakeholders. Deal with all general enquiries, explain procedures and appointment booking processes. Process appointment requests for the current day/future appointments from patients by telephone and in person and actively process home visit requests. Keep the Reception area and Waiting Room area noticeboards and leaflet dispensers tidy and free from obstructions and clutter. Using your own judgment and communication skills, ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner. Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, and ensure procedures are completed. Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery. Enter patient information onto the computer as required. Undertake any other additional duties appropriate to the post as requested by the partners or the practice manager. Person Specification Experience Essential Excellent written and oral communication skills. Ability to develop effective professional relationships with others. Develop own skills and knowledge and provide information to others to help their development. Skills of assessing and interpreting service user conditions with appropriate actions. Plan and organise complex activities or programmes requiring formulation and review. Able to use Word, PowerPoint, clinical systems, email and/or willingness to learn. The ability to multitask and mental adaptation to different unpredictable situations. Desirable Ability to drive. Punctual and flexible across hours of work when required. Qualifications Essential Relevant degree or equivalent level of training and experience. Evidence of a consistent pattern of learning from education, training, and experience. Desirable Qualification in health or social care allied profession.
