The role will work closely with YHN's Clinical Operations team to support the maintenance and development of the clinical services run by YHN. This will require leading new initiatives and services to establish clinically safe and efficient ways of working for newly introduced clinical services as well as the development and trial of improved processes within existing services. Reporting directly to the Clinical Services Manager, key working relationships will be built with YHN's Operations and Senior Leadership Team through the review and development of new, efficient services as well as with clinical colleagues to support their roles across YHN's operation. Main duties of the job

As a role-model to other clinicians working within YHN, the post-holder will exhibit clear values, behaviours and professionalism aligned with YHN's values and culture, encouraging consistency and aspirational standards across the wider clinical workforce. This unique role will have the opportunity to use their experience in the development of new processes whilst retaining a key role in the delivery of YHN's services by working directly across all of them to deliver a strong sense of ownership and variety in the role. This role offers a blend of hands-on clinical work and service development for the benefit of local patients' ability to access the clinical care they require. Job responsibilities

Key Responsibilities Assess, plan, develop, implement, and evaluate treatment programmes to promote health and well-being, and prevent adverse effects on health and well-being. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care. Deliver opportunistic health promotion using opportunities such as NHS health checks. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions. Work cohesively across all of YHN's clinical services to support direct service improvements and operational pilots in pursuit of clinically safe efficiencies and developments. Support colleagues in applying aspirational standards of care, professionalism and support to patients, consistently applied across YHN's clinical workforce. Delivering a quality service Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC). Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Deliver care according to NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation. Contribute to the assessment of other appropriate clinicians within YHN to allow rounded and effective feedback to drive high-standards and consistency across YHN's clinical team. Participate in the maintenance of quality governance systems and processes across the organisation and its activities. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required. In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local and national policies and initiatives as appropriate. Evaluate the patients' response to health care provision and the effectiveness of care. Support and participate in shared learning across YHN and wider practice network. Participate in the management, review and identify learning from patient complaints, clinical incidents and near-miss events utilising a structured framework (e.g., root-cause analysis). Participate in the performance monitoring review of the team, providing feedback as appropriate. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance. Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate. Person Specification

Skills

Excellent communication skills (written and oral) including ability to communicate sensitive information clearly. Ability to work as a team member and autonomously. Experience of the Web clinical systems (EMIS or SystemOne). Change-management skills and ability to support patients to change lifestyle. Experience

Clinical experience working in a GP practice. Teaching and Assessing experience. Knowledge of needs of patients with long-term conditions. Aware of accountability of own role and other roles in a nurse led service. Knowledge of health promotion strategies. Awareness of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Qualifications

First level Registered nurse. Qualification in cervical cytology. First Degree level qualification or working at this level. Travel Health and Immunisation Qualification. ARTP Qualification. Accredited training in long term conditions and/or sexual health. Other Requirements

Availability to work during the operating hours for YHN services including afternoon, evenings and weekends. Working hours between 09.00am-20.00pm. Working 5 days over 6. Saturday & Sunday are non-working days on week 1 and Friday and Sunday are non-working days on week 2. Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £27,373 to £39,105 a year depending on experience.
