Job summary

The post holder will act as an expert practitioner in occupational therapy practice and provide clinical expertise and professional support from within the Continuing Healthcare (CHC) service, to occupational therapy and other staff working with adults, children and young people with complex needs who require assessment and provision within the NHS Continuing Healthcare criteria.

They will provide specialist advice, undertake joint assessments with clinical staff in the field and will lead in the development of occupational therapy practice within this specialist area of community service provision.

Assessments will be undertaken for those with an identified Occupational Therapy need including adults, children and young people who live in residential nursing homes, private nursing homes, specialist out of county placements, own homes, supported living accommodation and also for adults with complex continuing care needs being discharged from hospital to their own or other accommodation.

Main duties of the job

To guide and direct the input of practitioners working with adults with Continuing Healthcare (CHC) needs in hospital and community. To have responsibility for improving the quality of OT practice in the area of CHC. To undertake 'one-off' assessments and interventions with less experienced therapists and other agency staff working in the hospitals and community in respect of adults, children and young people who are being assessed for eligibility for Continuing Healthcare. To demonstrate advanced knowledge relevant to the application of occupational therapy practice in the area of CHC. To be able to move and handle adults, children and young people with complex physical problems and disabilities, assess, prescribe and adjust specialist equipment. To assess and provide occupational therapy intervention in cases presenting a high degree of complexity. To independently plan, organise, lead and evaluate the occupational therapy service within CHC. To provide evidence based assessments and interventions and demonstrate high level clinical reasoning skills in implementing best practice; to explore innovations in practice and implement accordingly. To demonstrate an understanding and working knowledge of relevant legislation and local policies. To demonstrate an understanding and working knowledge of risk assessment and management. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and / or knowledge

Essential

Degree/Diploma in Occupational Therapy. Registered member of HCPC Postgraduate training in Fieldwork Education Desirable

MSc or post-graduate diploma in relevant area. Member of Professional Body Experience

Extensive post graduate clinical experience including manual handling, complex case management Experience of MDT/multi-agency working Experience of Research and Audit Proven leadership/management experience Desirable

Awareness of current professional issues. Experience of supervisory skills including appraisal. Skills

Professional confidence to represent CHC Service Ability to reflect and appraise own performance Demonstrate interpersonal and communication skills Highly effective written and verbal communication Autonomous practitioner with excellent personal organisational skills for caseload management, including time management and prioritisation skills. Demonstrable clinical reasoning skills, and able to make effective clinical decisions following assessment of highly complex cases. Computer literate Desirable
