Main duties of the job The PCPHS offers community-based assertive outreach work, as well as working directly into individual GP Practices. The successful applicant will be required to complete yearly physical health checks for those individuals identified via the GP network - this will include blood monitoring, checking blood pressure, height, weight, BMI and in some cases offering an electrocardiogram (ECG). The role requires clinical skills and competence in not only offering a comprehensive physical health assessment but also low level therapeutic intervention regarding the individual's health, lifestyle and access to appropriate health services including signposting to external agencies and services. The PCPHS functions as a clinical MDT from both within the organisation and with partner agencies and services to holistically and seamlessly meet the needs of the population of South Tyneside.

Person Specification Qualifications
GCSE maths and English at C or above/ equivalent adult learning qualification
Level 3 qualification (e.g. A-level, NVQ3) in Care or Level 2 qualification (e.g. GCSE, NVQ2) in care and will be expected to undertake the relevant level 3 qualification on appointment to post
Phlebotomy training completed
ECG training completed
Knowledge and Experience
Experience of working with service users with complex health needs within a community setting
Experience of working in Mental Health/Learning Disabilities health setting
Evidence of time management and autonomous working
Skills
Good interpersonal and communication skills
