A new and exciting development role has arisen within Community Nursing and the Carmarthenshire Intermediate Care Multi-Disciplinary Team in Hywel Dda University Health Board.

We are looking for an enthusiastic individual to join our Intermediate Care Team as a Band 8a- Community Advanced Nurse Practitioner.

The successful candidate will work alongside the county’s Intermediate Care and urgent care multi-disciplinary professionals (comprising intermediate care GPs, therapists, acute response team nurses, health care assistants, and social workers). The aim is to provide advanced clinical assessment and treatment to patients presenting with a Differentiated/Undifferentiated medical and/or intermediate care/nursing need in their own home.

They will demonstrate safe clinical decision-making and expert care, acting within their professional boundaries and qualifications.

The post holder will attend home visits to include residential and nursing homes, where they will liaise with the intermediate care team GP, staff, family, and other stakeholders in developing a suitable patient-centered management plan. This will include writing care plans and management plans to be shared with other providers including the Out of Hours (OOH) service.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
