INTERNAL APPLICANTS ONLY 1 x 36 hours, 52.14 weeks per year, Permanent An exciting opportunity has arisen for a Full Time equivalent Care Co-ordinator to join adults and older people social work services. Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other disciplines within the team, which will comprise Occupational Therapists, Social Workers, community care assistants, and technology staff. Adult and Older People Social Work is responsible for the assessment for new community services and the review of existing services. This post will also see the successful applicant support community hubs across the authority, working with a number of community partners and key stakeholders. Successful applicants must have the ability to work as part of a team, respond to the demands of a busy service, follow procedures, and possess excellent communication and report writing skills. They must be able to travel throughout West Lothian and hold a recognised qualification in social care. The skills and knowledge applicants must have are: SVQ 3 and an HNC in Social Care or equivalent. It is essential for candidates to have relevant assessment and care management experience in an adults or older peoples social work practice team. This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
