What You Will Do:
Conduct community clinics and home visits around Greater Preston, Chorley, and South Ribble (PR1-PR26) to provide dietary advice and support.
Implement nutrition support strategies, including home enteral feeding and basic diabetes management.
Design and deliver personalised weight management programmes.
Collaborate with multidisciplinary teams in community hospitals to enhance patient care.
Educate patients and their families on maintaining a healthy diet and lifestyle.
What Experience is Required:
A minimum of 2 years experience as a Community Dietitian.
Home enteral feeding experience, nutrition support, diabetes management, and weight management are desirable.
Must be HCPC registered to ensure the highest standards of practice.
