Community Heart Failure Clinical Nurse Specialist

The role of the Heart Failure Nurse within the community has been established to meet the needs of people with heart failure in the Surrey locality. The aim is to optimise the management of people with heart failure, reducing unnecessary hospital admissions and providing a link between the GPs and specialist teams. The post holder will offer education and support to health care professionals, people with heart failure, and those who care for them. Main Duties of the Job

Responsible for communicating highly sensitive information and bad news to patients and carers in an empathetic and supportive manner, empowering the patient to manage their condition (e.g., development and impact of complications, effect on activities of daily living). Responsible for teaching patients and their carers to manage their heart failure to the best of their abilities, aiming to prevent short-term and long-term complications, either one-to-one or in groups. Communicates effectively with patients and carers to ensure a clear understanding of heart failure and its management, providing and receiving complex, sensitive information, requiring developed motivational, empathetic, and reassurance skills. Acts as an advocate for patients, particularly in residential homes, ensuring they receive the understanding and facilities needed to manage their heart failure. Responsible for maintaining and developing effective joint working relationships with nursing and medical colleagues throughout partnership working. Responsible for providing advice and information on heart failure, that may be complex, to medical and nursing colleagues and the general public. About Us

Experience

Up-to-date knowledge of current clinical and professional issues. BLS. Knowledge and Skills

Good organisational skills. Good time management. Ability to work independently and unsupervised. Good computer and keyboard skills to use Excel. Good communication skills, both written and verbal. Counselling skills. Qualifications

RGN or equivalent HCPC qualification. BSc or equivalent qualification in profession. Advanced Clinical Assessment skills. ENB 998 Teaching and assessing or equivalent. Independent non-medical prescriber V300. Qualified for 5 years with 2 years cardiac nursing experience. Evidence of CPD at Level 7. Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £48,270 to £54,931 a year inclusive of 5% High Cost Area allowance per annum pro rata.
