Main Duties of the Job Are you a clinician who has the passion, drive and ability to deliver high quality person-centred compassionate care in accordance with the Trust's organisational vision and values?
As an NMC registered nurse, qualified in physical health assessment, you will have significant experience working with patients who have heart failure. Strong communication, organisational skills, and a flexible approach are essential to work collaboratively with patients, cardiac key partners and the wider MDT to facilitate the best possible patient experience and outcomes.
You will demonstrate the ability to effectively and efficiently assess and examine, plan, provide and evaluate evidence-based nursing care for patients.
You will demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively in an autonomous role.
You will have a commitment to professional development and contribute to a culture of continuous improvement, supporting clinical mentoring.

Job Responsibilities

To be professionally accountable and responsible for all aspects of own work.
To establish and maintain effective working relationships both within the Trust and with external organisations in order to meet service requirements and support and demonstrate the Trust's equality values.
To assess patients and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment/care/support for individuals, families and carers.
To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available.
Person Specification Qualifications & Training
Degree in nursing or equivalent
Registration with NMC
Post Graduate studies in relevant nursing field such as; end of life care, long term conditions/palliative care or related/relevant subject
Venepuncture
Post qualification courses in nursing
Attendance at relevant networks e.g. Special Interest Groups
Up to date field work/pre-registration student training. i.e. Cardiac/ Heart Failure specialism areas.
Modules at Masters level
Non-medical Prescribing V300 or working towards
Evidence of further clinical study
Experience
Relevant experience to the role including assessment and management of long term conditions
Ability to manage time and caseload effectively
Supporting service users, families and carers in dealing with Heart Failure and its associated complications. Working in a multidisciplinary team
Experience of negotiation and conflict management in clinical situations
Experience in leading and supporting junior staff
Experience of involvement in service improvement
